In order to obtain the Chifa cards for health insurance, we invite the students of the school (who didn’t have the Chifa card) to contact the administration as soon as possible and present the following documents:

1) Two Photo

2) A birth certificate

3) A photocopy of the blood group certificate

4) A filled Chifa form (download the form).
